- Please read carefully the information and instructions printed in this packet.
- Be certain to include all attachments.
- Attach all extra papers securely by stapling or otherwise fastening them to the packet.
- Include your full name on the upper left corner of all pages, including attachments.
- Make sure all portions of the application are completed. Incomplete applications will not be considered.
- Sign under "Part VII: Signature" located at the end of the application.
- Return this application as soon as possible, but no later than March 2, 2009.
